Обследовались пациенты, перенесшие длительные абдоминальные операции (панкреатодуоденальная резекция, гемигепатэктомия, гемиколэктомия, реконструктивно-пластические операции на желчных протоках), а также пациенты с гнойно-септическим процессом (перитонит, панкреонекроз), имевшие при поступлении в отделение интенсивной терапии признаки фибринолитическо-го варианта ДВС-синдрома: повышение уровня продуктов деградации фибрина/фибриногена (ПДФ) выше 10 мкг/мл, укорочение времени эуглобулинового лизиса ниже 180 мин, повышение фибринолитического потенциала (ФП) на фоне рыхлого сгустка по данным электрокоагулограммы (выше 1,1 усл. ед.).</p></sec><sec><title>Результаты</title><p>Результаты. Использование ингибиторов протеаз при фибрино-литическом варианте ДВС-синдрома в раннем послеоперационном периоде позволило снизить потребность в гемотрансфузиях (на 47—67%), в переливании свежезамороженной плазмы (на 47—68%) и уменьшить количество геморрагических потерь по дренажам (на 57—73%). При использовании малых доз трасилола достоверной разницы с исходным уровнем не получено.</p></sec><sec><title> </title><p> </p></sec><sec><title>Заключение</title><p>Заключение. При наличии у пациента фибринолитического варианта ДВС-синдрома препараты апротинина должны быть неотъемлемой частью интенсивной терапии. Сравнение эффективности трасилола и апротекса свидетельствует, по крайней мере, о равноценности этих двух препаратов апротинина в условиях применения эквивалентных доз. Применение в хирургии «малых доз» апротинина неприемлемо, так как не позволяет достаточно быстро скорректировать гемостазиологические нарушения и снизить тем самым кровопотерю.</p></sec><sec><title> </title><p> </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to comparatively evaluate the efficiency and safety of trasylol and aprotex used in the treatment of the fibrinolytic type of disseminated intravascular coagulation (DIC) syndrome after long abdominal operations.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The authors examined patients undergone long abdominal operations (pancreatoduodenal resection, hemihepatectomy, hemicolectomy, reconstructive operations on the bile duct) and those with a pyoseptic process (peritonitis, pancreatonecrosis) who had on admission to the intensive care unit the signs of fibrinolytic DIC syndrome: elevated levels of fibrin/fibrinogen degradation products &gt; 10 ^g/ml, shorter euglobulin lysis &lt; 180 min, and increased fibrinolytic potential (&gt; 1.1 conventional unit) in the presence of a porous clot, as evidenced by an electrocoagulogram.</p></sec><sec><title>Results</title><p>Results. The use of protease inhibitors in the fibrinolytic type of the DIC syndrome in the early postoperative period could reduce needs for blood transfusion by 47—67%, fresh frozen plasma transfusion by 47-68%, and drainage losses by 57—73%. As compared with the baseline values, there was no significant difference with the use of small-dose trasylol.</p></sec><sec><title>Conclusion</title><p>Conclusion. In the fibrinolytic type of DIC syndrome, aprotinin should be an integral part of intensive care. Comparison of the efficacy of trasylol and aprotex suggest the equivalence of both aprotinin drugs used in equivalent doses.
