Оптимизировать результаты лечения больных с острой дыхательной недостаточностью вследствие ушиба легких путем применения методов неинвазивной масочной респираторной поддержки.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включен 31 больной с тяжелой тупой травмой груди, множественными переломами ребер и гипоксемической ОДН. Этим больным проводилась вспомогательная вентиляция в режиме CPAP+PSV через лицевую или носовую маску. Физиологические показатели регистрировались поэтапно в процессе проведения неинвазивной масочной вентиляции легких (НМВЛ). В контрольную группу включены 25 больных с аналогичной тяжестью травмы и ОДН, получавших ИВЛ через эндотрахеальную трубку.</p></sec><sec><title>Результаты</title><p>Результаты. У 67,7% больных исследуемой группы НМВЛ была эффективна в улучшении оксигенации и показателей внешнего дыхания, не оказывая отрицательного влияния на гемодинамику. У этих больных удалось избежать эндотрахеальной интубации и ИВЛ. При сравнении исследуемой и контрольной групп выявлено значительное снижение числа вторичных пневмоний в группе НМВЛ, а также сроков лечения. Использование масочной вентиляции позволило снизить летальность с 44% в контрольной группе до 9,7% в группе НМВЛ.</p></sec><sec><title>Заключение</title><p>Заключение. Применение НМВЛ у больных с гипоксемической ОДН на фоне ушиба легких улучшает легочную функцию и у большей части больных позволяет избежать эндотрахеальной интубации и ИВЛ, а следовательно, связанных с ними осложнений. При применении масочной вентиляции укорачивается и упрощается лечение больных, значительно снижается летальность. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To optimize the results of treatment in patients with acute respiratory failure (ARF) due to lung contusion, by using the methods of non-invasive mask respiratory support.</p></sec><sec><title>Materials and methods</title><p> Materials and methods. The study covered 31 patients with severe blunt chest injury, multiple costal fractures, and hypoxemic ARF. The patients underwent assisted ventilation (AV) in the CPAP+PSV mode through a facial or nasal mask. Physiological parameters were recorded during non-invasive mask ventilation (NIMV) in the stepwise fashion. A control group comprised 25 patients with the similar severity of injury and ARF who were given conventional AV.</p></sec><sec><title>Results</title><p>Results. In 67.7% of the study group patients, NIMV was effective in improving oxygenation and external respiration, without exerting negative hemodynamic effects. Endotracheal intubation and AV could be avoided in these patients. Comparison of the study and control groups revealed a significant reduction in the incidence of secondary pneumonias in the NIMV group and in the duration of treatment. Mask ventilation could decrease mortality from 44% in the control group to 9.7% in the NIMV group.</p></sec><sec><title>Conclusion</title><p>Conclusion. NIMV applied to patients with hypoxemic ARF in the presence of lung contusion improves pulmonary function and, in the bulk of patients, allows endotracheal intubation and AV and consequently their associated complications.
