Изучение динамики клиниколабораторных показателей с целью создания индивидуальных временных шкал у новорожденных в критическом состоянии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследовали 229 новорожденных детей со сроком гестации от 25 до 42 недель. Состояние детей оценивали ежедневно в течение всего периода наблюдения в ОРИТ по 105 признакам. Из новорожденных, включенных в исследование, 6 пациентов умерло, 223 выжили. Исследовали 24 количественных признака, подверженных по результатам однофакторного дисперсионного анализа влиянию факторапеременной «Исход», отражающей ранние исходы критических состояний.</p></sec><sec><title>Результаты исследования</title><p>Результаты исследования. Установлено, что основными показателями, отражающими тяжесть состояния пациента и определяющими исход критического состояния, являются такие характеристики кислородного статуса, как содержание кислорода в артериальной крови, индекс системной доставки кислорода, индекс оксигенации, индекс системного потребления кислорода и напряжение кислорода в артериальной крови. Выявлено, что у новорожденных с благоприятным исходом критического состояния имеет место стойкое увеличение показателей кислородного статуса в динамике с одновременным снижением индекса оксигенации, в то время как при неблагоприятном исходе имеет место выраженное снижение показателей, отражающих кислородный статус организма при значительном увеличении индекса оксигенации. Установлено, что показатель напряжения артериального кислорода не соответствовал референтным значениям даже при благоприятном исходе критического состояния. Продемонстрировано, что снижение индекса системного потребления кислорода в группе неблагоприятного исхода отражает снижение скорости метаболических процессов и свидетельствует о терминальном состоянии пациента.</p></sec><sec><title>Заключение</title><p>Заключение. Оценка потребления кислорода тканями в динамике дает возможность оценки эффективности проводимых мероприятий интенсивной терапии, создания индивидуальной временной шкалы и прогнозирования исхода критического состояния у конкретного пациента.</p></sec></abstract><trans-abstract xml:lang="en"><p>Prediction of neonates' critical states outcome depending on lab test findings and clinical manifestations is one of the most important and difficult problems of modern critical care medicine.</p><sec><title>Aim of the study</title><p>Aim of the study. To evaluate changes in lab test findings and clinical manifestations in order to create individual timelines for critically ill neonates.</p></sec><sec><title>Material and methods</title><p>Material and methods.We examined 229 neonates with a gestational age of 25—42 weeks. Babies' health was assessed daily throughout their ICU stay using 105 parameters. Six patients enrolled in the study died, 220 enrolled patients survived. We analyzed 24 quantitative attributes affected by variable factor «Outcome» (according to the univariate variance analysis) that reflects early outcomes of a critical state.</p></sec><sec><title>Results</title><p>Results. The study demonstrated that the main characteristics reflecting the severity of patient's condition and determining the outcome of a critical state are such parameters of the oxygenation status as total blood oxygen concentration, oxygen delivery index, oxygenation index, oxygen consumption index, and arterial oxygen extraction tension. The study demonstrated that neonates with favorable outcomes presented a steady increase in the oxygenation status parameters with concurrent decrease in the oxygenation index. At the same time, neonates with unfavorable out comes presented a steady decrease in the oxygen status parameters along with a significant increase in the oxygenation index. We determined that the arterial oxygen extraction tension did not comply with the reference limits even in case of a favorable outcome. The study demonstrated that a decrease in the oxygen consumption index in the unfavorable outcome group reflects a decrease in the metabolic rate in patient and indicates a terminal state of the disease.</p></sec><sec><title>Conclusion</title><p>Conclusion.
