СВР, возникающая у пациентов в интра- и послеоперационном периодах кардиохирургических вмешательств, является естественной реакцией организма, однако в ряде случаев имеет место чрезмерная генерация провоспалительных импульсов, изменяющая характер воспаления и усугубляющая тканевое повреждение. Массивное высвобождение медиаторов воспаления приводит к дисфункции различных органов и систем и может служить одной из основных причин летальных исходов в послеоперационном периоде. Существуют свидетельства роли системного воспаления в патогенезе атеросклероза. Таким образом, оценка предрасположенности пациентов к характеру развивающегося в послеоперационном периоде системного воспалительного ответа поможет прогнозировать риск развития послеоперационных осложнений воспалительного генеза или тяжесть их течения, и подобрать специфическую терапию для конкретного пациента. В данном обзоре анализирована информация о роли полиморфизма генов, кодирующих цитокины и белки, вовлеченные в патогенез системного воспалительного ответа, в формировании индивидуальной предрасположенности к развитию системного воспаления неинфекционного генеза у пациентов, перенесших кардиохирургические вмешательства, а также в модификации его тяжести и последствий.</p></abstract><trans-abstract xml:lang="en"><p>The systemic inflammatory response syndrome is a typical systemic reaction to tissue injury and an important factor for structural and functional regeneration of the damaged tissue. The systemic inflammatory response syndrome developed in patients in the postoperative period after cardiac surgeries is a natural body reaction; however, in some cases there is an excessive generation of pro-inflammatory factors that can change the nature of inflammation and worsen the tissue damage. The massive release of inflammatory mediators leads to dysfunction of various organs and systems and can become one of main causes of lethal outcomes in the postoperative period. There is evidence of the contribution of the systemic inflammation to the pathogenesis of atherosclerosis. Therefore, an assessment of patient’s susceptibility to the systemic inflammatory response may contribute to predicting disease risks and severity as well as choosing a specific therapy for a given patient.
