Одним из компонентов защиты ГМ при этих операциях может являться умеренная контролируемая общая гипотермия.</p><sec><title>Цель исследования</title><p>Цель исследования. Изучить нейропротективные возможности умеренной системной гипотермии при проведении каротидной эндартерэктомии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включили 59 пациентов. Всем проводили КЭАЭ в условиях сочетанной анестезии. Пациентов разделили на 2 группы: основную (гипотермии, n=33) и сравнения (нормотермии, n=26). В обеих группах выполняли стандартные мероприятия по профилактики ишемии ГМ. В основной группе дополнительно проводили умеренную системную гипотермию в целевом температурном диапазоне 34–35°С. Когнитивные функции оценивали до операции, на 2-е, 5-е, 30-е сут послеоперационного периода при помощи нейрокогнитивных тестов. Статистическую обработку данных проводили в программе IBM SPSS Statistics.</p></sec><sec><title>Результаты</title><p>Результаты. Частота развития когнитивных расстройств в группе гипотермии составила 21,1%, в группе нормотермии — 26,9%. Послеоперационные когнитивные нарушения в группе нормотермии наблюдали чаще: на 5-е сут — у 15,38%, а на 30-е сут — у 11,5% пациентов против 12,1% и 6,1% в группе гипотермии, соответственно (p&lt;0,05).</p></sec><sec><title>Заключение</title><p>Заключение. Выявили нейропротективные возможности гипотермии в виде снижения выраженности когнитивных расстройств у пациентов основной группы. Необходимы дальнейшие исследования с целью выявления пациентов групп риска, у которых данный вид нейропротекции будет иметь наилучшие результаты. Необходимо также определение оптимальной методики гипотермии.</p></sec></abstract><trans-abstract xml:lang="en"><p>Prevention of brain injury during carotid endarterectomy (CEA) remains a significant challenge. Moderate controlled systemic hypothermia may serve as a potential neuroprotective measure during these procedures.</p><sec><title>Aim of the study</title><p>Aim of the study. To investigate the neuroprotective effects of moderate systemic hypothermia during CEA.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Fifty-nine patients undergoing CEA under combined anesthesia were included. Patients were divided into two groups: the hypothermia group (N=33) and the normothermia control group (N=26). Both groups received standard measures to prevent cerebral ischemia. The hypothermia group received additional moderate systemic hypothermia aimed at a temperature range of 34–35°C. Cognitive function was assessed preoperatively and at 2, 5, and 30 days postoperatively using neurocognitive tests. Statistical analysis was performed with IBM SPSS Statistics.</p></sec><sec><title>Results</title><p>Results. The incidence of cognitive impairment was 21.1% in the hypothermia group and 26.9% in the normothermia group. Postoperative cognitive impairment was more common in the normothermia group: 15.38% on day 5 and 11.5% on day 30 postoperatively compared to 12.1% and 6.1% in the hypothermia group (P&lt;0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion. This study demonstrated the neuroprotective effects of hypothermia, manifested by a reduced severity of cognitive impairment in the hypothermia group.
