Сравнить влияние рестриктивного и либерального подхода к инфузионной терапии на длительность искусственной вентиляции легких (ИВЛ), общее время пребывания в ОРИТ, потребность в кардиотонической и вазопрессорной поддержке в постперфузионном периоде и в течение 1-х суток после операции у детей раннего возраста, оперируемых по поводу врожденных пороков сердца (ВПС).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Выполнили рандомизированное, проспективное, одноцентровое исследование. В него включили пациентов с врожденными пороками сердца (ВПС), разделенных на две группы: в 1-й группе пациенты получали инфузионную терапию по рестриктивному протоколу (группа РП, n=65) — 8 мл/кг/ч, во 2-й группе использовали либеральный протокол (группа ЛП, n=67) — 16 мл/кг/ч. Изучали динамику метаболических нарушений, продолжительность респираторной поддержки, постоперационную прибавку в весе и сроки пребывания больных в ОРИТ.</p></sec><sec><title>Результаты</title><p>Результаты. Продолжительность ИВЛ и общее время нахождения в ОРИТ были больше в группе РП по сравнению с группой ЛП: 14±5 ч vs 10±3 ч (p=0,035) и 23±2 ч vs 27±4 ч (p=0,036), соответственно. При этом среднее значение процентного прироста веса в группе ЛП составило 2,00%, vs 0,32% в группе РП (р=0,001). В обеих группах не отмечали значимых метаболических и электролитных нарушений, за исключением уровня ионов K в группе ЛП.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты противоречат данным, полученным при исследовании взрослых пациентов с ВПС. При либеральном подходе к инфузионной терапии у детей раннего возраста продолжительность ИВЛ и общее время нахождения в ОРИТ оказались меньше, чем при рестриктивном. Дети раннего возраста наиболее чувствительны к объему инфузионной терапии, потребность в преднагрузке жидкостью у детей выше, чем у взрослых.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim: to compare the effects of restrictive versus liberal fluid therapy on the duration of mechanical ventilation (MV), total intensive care unit (ICU) stay, and the need for inotropic and vasopressor support during the post-perfusion period and the first 24 hours after surgery in young children undergoing surgical correction of congenital heart defects (CHD).</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. A prospective, randomized, single-center study included pediatric patients (toddlers) with CHD who were assigned to one of two groups. Group 1 received fluid therapy according to a restrictive protocol (RP group, N=65) at 8 mL/kg/h, while group 2 received therapy according to a liberal protocol (LP group, N=67) at 16 mL/kg/h. The study evaluated the dynamics of metabolic disturbances, duration of ventilatory support, postoperative weight gain, and total ICU stay.</p></sec><sec><title>Results</title><p>Results. Mechanical ventilation time and total ICU stay were longer in the RP group compared to the LP group: 14±5 hours vs. 10±3 hours (P=0.035) and 27±4 hours vs. 23±2 hours (P=0.036), respectively. Mean postoperative weight gain in the LP group was 2.00% vs 0.32% in the RP group (P=0.001). No clinically significant metabolic or electrolyte disturbances were observed in either group, except for elevated K ion levels in the LP group.</p></sec><sec><title>Conclusion</title><p>Conclusion. These findings contradict previously reported data in adult population with CHD. In toddlers, a liberal approach to fluid therapy resulted in shorter duration of ventilation and ICU stay compared to a restrictive approach.
