Нарушения системы гемостаза у пациентов в остром периоде изолированной черепно-мозговой травмы (обзор)
Аннотация
Острая изолированная черепно-мозговая травма (ЧМТ) часто связана с возникновением нарушений системы гемостаза, что может сопровождаться геморрагическими и ишемическими проявлениями в веществе головного мозга, в связи с чем нормальное функционирование свертывающей системы крови имеет критическое значение. Понимание патофизиологических механизмов данного явления может помочь адекватной профилактике вторичного повреждения головного мозга. Ранее, развитие синдрома диссеминированного внутрисосудистого свертывания (ДВС-синдрома) являлось общепринятой гипотезой механизма нарушений свертывающей системы при ЧМТ. Однако, за последние десятилетия появлялись новые данные о ключевой роли тканевого фактора, системного воспалительного ответа, тромбоцитопатии, эффекта протеина C в возникновении данной коагулопатии. Целью данного обзора литературы являлся предоставление новых данных о конкретных патофизиологических механизмах, лежащих в основе коагулопатии после ЧМТ.
